Parkinson’s Disease
By Dr. HK’s Neuro and Parkinson’s Clinic, Bangalore
Parkinson’s disease (PD) is a progressive neurological disorder that primarily affects movement and coordination. It results from the gradual loss of dopamine-producing nerve cells in the brain, particularly in an area called the substantia nigra, which plays a critical role in controlling movement.
What Causes Parkinson’s Disease?
While the exact cause remains unknown, several factors contribute to PD including:
- Genetic mutations, although rare, can predispose some individuals
- Environmental exposures such as pesticides, toxins, and certain drugs
- Aging, which increases risk as dopamine-producing cells naturally decline
Symptoms to Watch For
Parkinson’s disease manifests both motor and non-motor symptoms:
- Motor Symptoms:
- Tremors, usually beginning in one hand and occurring at rest
- Muscle stiffness or rigidity, causing pain and limiting movement
- Bradykinesia or slow movements making everyday activities difficult
- Postural instability and balance problems leading to falls
- Non-Motor Symptoms:
- Sleep disturbances, mood disorders such as depression and anxiety
- Cognitive changes and dementia in advanced stages
- Fatigue, constipation, and impaired sense of smell
Diagnosis
Diagnosis is mainly clinical, based on symptoms and neurological examination. Imaging and specialized tests, including dopamine transporter scans, may support diagnosis but are not definitive.
Treatment Options
While there is no cure for Parkinson’s, treatments focus on symptom management to improve quality of life:
- Medications:
- Levodopa is the most effective drug, converted to dopamine in the brain to improve motor function
- Dopamine agonists mimic dopamine’s effects
- Other drugs like MAO-B inhibitors and anticholinergics help manage specific symptoms
Medication regimens are personalized and may change over time as symptoms progress.
- Surgical Treatments:
- Deep Brain Stimulation (DBS) offers significant relief from motor symptoms when medications are insufficient.
- Rehabilitation:
- Physiotherapy, occupational therapy, and speech therapy improve mobility, daily functioning, and communication
